I offer this as our next amendment to the Constitution:
Except as provided for elsewhere in this Constitution, neither house of Congress shall conduct any business except by a simple majority of those present and voting. The term "business" shall include, but not be limited to, any question, motion, resolution, ruling or vote.

I would prefer at least a quorum be present for any voting. "Those present" could mean just a handful of the majority party meeting unannounced (or some of the announcements getting "lost" in transit."
I think a non-delegation clause would be more appropriate fix.

You should add :
"The Speaker shall read aloud any Bill that is before the house, before a vote is taken."
